DIAGNOSIS OF TB IN INDIA

Current tools and strategies for diagnosis of TB in India are inadequate. The weak systems for lab-based diagnosis of TB have contributed to (1) underdiagnosis of disease, leading to high mortality and continued transmission; (2) overdiagnosis of disease, leading to unnecessary treatment and inappropriate resource utilization of the healthcare programmes and (3) delayed diagnosis of drug resistance, leading to morbidity and more transmission. HIV infection too dramatically increases the incidence and mortality risk of TB. There is a clear need for introduction, development and effective implementation of new cost-effective tools that contribute to improvement in patient-centred outcomes and public health in India. In a country like ours, the greatest challenge is to translate technical advances into sustainable improvements in public health in settings hardest hit by TB.

TB Management Programs

The introduction of the various policies in the TB management program is an important need in India however the implementation of these policies is the most important. The National Tuberculosis Programs (NTP) must be adopted and scaled-up to the recent trends in TB. The challenges to effective NTPs may include insufficient information for scale-ups by the global policy recommendations, intricate policies for decision making, inadequate political commitment, inadequate resources and many more. The revised national tuberculosis control program (RNTCP) aims at reducing the number of deaths due to TB including those by HIV also to less than 1 million. The optimal steps required to overcome these challenges are the need of the hour. Each one of us must take a pledge to fight against TB and push India towards a TB free country.

Tuberculosis Control Program- Is India Doing Enough?

The prevalence of TB in India is so high that over 40% of the Indian population is suffering from TB, with 2 deaths in every 3 minutes due to TB. India is facing the burden of both pulmonary and extra-pulmonary TB. The number of multidrug resistant TB patients are increasing in numbers in India yet the MDR TB management is unsatisfactory. Extensively drug resistant TB is emerging with the potential of an incurable disease. Immediate action plan that needs to be undertaken are improvement of pathological services, drug susceptibility testing for mycobacterium species and early start of the specific TB- Treatment method.

Questions that arise now are, are we doing enough to control this disease effectively? This TB day, 2014 what is there that is still to be known about TB control in India? How can we help save a life every 3 minute? All these questions will be rightly addressed in the TB symposium on Breakthrough in Diagnosis and Effective Treatment of TB conducted Dr. Lal Path Labs on Sunday 30th of March 2014 between 11 AM to 1 PM. This webinar will be webcast from Hall Viceregal, Hotel Claridges, New Delhi.

A Distant Dream- TB Free India

India is among these high TB burden countries with a whopping 26% of the world TB patients. With the emergence of newer strains of Mycobacterium Tuberculosis, some of which are extensively drug resistant, TB has become a challenge to treat, requiring an immediate and strong action against the disease for the wellbeing of humanity.  India, in 1948, introduced bacille Calmette-Guerin (BCG) vaccine in its mass immunization program. This program has however failed to reach its expected potential. With the introduction of Directly Observed Treatment, Short Course (DOTS) in India in 1997, a major step towards TB elimination has been taken by the government on a large scale.
