Current tools
and strategies for diagnosis of TB in India are inadequate. The weak systems
for lab-based diagnosis of TB have contributed to (1) underdiagnosis of
disease, leading to high mortality and continued transmission; (2)
overdiagnosis of disease, leading to unnecessary treatment and inappropriate
resource utilization of the healthcare programmes and (3) delayed diagnosis of
drug resistance, leading to morbidity and more transmission. HIV infection too
dramatically increases the incidence and mortality risk of TB. There is a clear
need for introduction, development and effective implementation of new
cost-effective tools that contribute to improvement in patient-centred outcomes
and public health in India. In a country like ours, the greatest challenge is
to translate technical advances into sustainable improvements in public health
in settings hardest hit by TB.
